The first time you make payment either online or at the clinic you will be registered as a customer. The personal information we record is name, address, phone number and possible e-mail.
We register your personal information for the sole purpose of ensuring the best possible service to you and possible partner, and to ensure that accounting transactions are correct.
Information in our payment system will not be passed on to any third party.
You always have the right to object to registration, and the right to inspect our records.

All prices in our price list are in Danish Kroner (DKK) and VAT free. VAT is not calculated of the clinic's services.
Prices are excl. possible transaction fee in connection with payment through banks, online banking or by credit cards.
The Health Record Consultation
For the health record consultation you must have paid for the interview and submitted your completed journal at least 10 day before the interview is scheduled.

Booking of the health record consultation is financially binding. This means that you can only expect a refund if we are able to pass on your appointment to someone else.
Insemination
Once you have booked an appointment for insemination, we count on you to come.
However, should it happen that you regret your decision, or something happens which prevent you from showing up, naturally, it is quite okay that you cancel your appointment. You will not be charged if only you notify us the day before or at the very latest, submit a message on our answering maschine before 8 a.m. on the very day.
This gives us an opportunity to pass on the appointment to someone else.
For later cancellation we will unfortunately have to charge.
Being late
It rarely happens that someone comes in late, but if it does, it has implications in relation to the insemination. The donor sperm we use are thawed and purified an hour before your arrival. After thawing the semen donor insemination should take place within the first hour.
If you are late, we must know at least 2 hours before the scheduled time, otherwise we will have to bill you for the wasted insemination because sperm which has waited more than an hour, is not always useful.
If you are late and wish us to prepare a new sperm cleansing, we will have to charge you for 2 inseminations - the one you were late for and the new we must prepare.
Reservation of donor sperm
Once you become pregnant, in most cases it is possible to reserve sperm from the same donor for a subsequent pregnancy. Sperm banks recommend that you reserve sperm for at least 8 inseminations, but of course you decide for yourself how many servings you wish to reserve. You buy and pay for the portions of donor sperm, you wish to reserve. The prices of the various sperm types, freight and depot rental is reflected in our price list.
Payment of reservation of donor sperm, freight and depot rental are paid in advance.
If you are in need for guidance, Stork Clinic will gladly assist you.
If you are not using all your reserved donor sperm, you may ask us to sell the sperm to other women. If we succeed in finding another user of the sperm, 80% of the retail prise is refunded. Administration fee and depot rental will not be refunded.
Payment of the health record conversation, possible reservation of sperm, sending of network messages and signing up for the Stork get-together should be done in advance. Inseminations are paid for at the clinic.
Payments in advance can be done through our online payment module on our website, via banking or online banking, or by sending a crossed check addressed to Stork Clinic.
Payment on our website
We accept online payment with the following credit cards: Visa, Visa Electron, Maestro or Mastercard. We use a secure server to process all online credit card orders.
Entered data is encrypted and can not be read by others.
The secure connection is indicated by a small padlock at the bottom right of your browser. Stork Clinic has at no time access to your credit card information.
Stork Clinic uses a payment service from ePay, approved by PBS.
You will be charged a Credit Card transaction fee not exceeding fees set by PBS.
The fee will be disclosed before you accept payment.
Upon completion of the payment transfer you automatically receive a email confirmation in which the transfer amount is specified.
We recommend that you print out your receipt and bring it to your visit at the clinic.
Transfer to our bank account
It is a prerequisite for the Health record consultation via telephone, that we have registered your payment in advance. Hence, the following data should appear very clearly from the information we receive from your bank:
If this information does not appear on your bank statement, we are unable to register your payment and thus cannot conduct the interview.
If your partner conduct the payment, your name must clearly appear on the bank statement. Note that banks might charge you with a transaction fee for banks transfers. Normaly the fee range from 30,- to 70,- DKK.
If you wish to pay for your insemination via bank transfer or online banking, it must be done in advance. The above data (1+2) must be included in the bank statement.
Always remember to bring documentation for your payment if you have paid in advance.
Please note that it it might take a week before a bank transfer is registered in our bank account.
